Someone is coming home from the hospital

Equipment, follow-ups, and prescriptions settled before discharge day.

Hospital-to-Home Transition

Video or in person

Everything coordinated before discharge, so coming home is organized from the first hour instead of improvised for two weeks.

What you get: review of the discharge instructions, equipment ordered and confirmed, follow-up appointments booked, provider contact list assembled, home readiness checked, and a written transition plan for the first two weeks.

Why it matters: Small things get missed in the first days home: a prescription nobody filled, a follow-up nobody booked, a shower with no grab bar. This service is meant to catch them before discharge.

Timing: most useful in the 48 hours before discharge. Call as soon as a discharge date is mentioned.

$250Included when Gilgal home care starts within 30 days of the stay

The family needs to decide together

Who provides care, how it is paid for, and who does what.

Family Care Planning

Video or in person

A written care plan built around your person: their routines, their preferences, their actual home, and what your family can realistically sustain.

What you get: a documented plan covering daily care needs, safety, medications, appointments, emergency contacts, and who does what. Written in plain language you can hand to a caregiver, a sibling, or a hospital.

Why it matters: Care that lives in one person's head falls apart when that person gets sick or has to be away. A written plan does not depend on any one person.

Best booked as: a 60-minute session. Thirty rarely covers a full plan.

Family Care Conference

Video or in person

A 90-minute family meeting with someone who knows what care costs and how it works.

What you get: a pre-call with the organizing family member to set the agenda, a 90-minute conference in person or on video, and a written record of what was decided and who agreed to do what.

Why it works: One sibling believes round-the-clock care runs $3,000 a month. Another is certain Medicare covers it. Neither is right, and they have been arguing about it for six weeks. Once real numbers, real options, and real timelines are on the table, a lot of the conflict goes away.

What I bring: what care actually costs in this region, what Medicaid and waiver programs do and do not cover, what a direct care worker may legally do, what tends to break down at 2am when nobody has planned for it, and what other families in this position have chosen.

What this is not: I am not an attorney and I am not a certified mediator. I do not decide who is right, and the written record is a summary of your family's decisions. It is not a legal agreement and it binds no one.

When I will refer you elsewhere: contested guardianship or power of attorney, suspected financial exploitation, estate or inheritance disputes, and questions about whether someone still has capacity to decide. Those need an elder law attorney or a physician. If the conflict runs deeper than information, a certified elder mediator is the right call and I will say so.

I want someone with me at the appointment

I take notes and ask your questions, by phone or video anywhere in Pennsylvania, or in person in the five counties.

Virtual Appointment Support

Anywhere in Pennsylvania

I join your appointment by phone or video: a telehealth visit, or an in-person visit with me on speakerphone in the exam room.

What you get: a 15-minute prep call beforehand to build your question list, presence for the appointment, and a written summary the same day covering what was said, what was decided, and what happens next.

Why it works: No travel means no drive time to pay for, and it is available anywhere in Pennsylvania rather than just our five counties. Most appointments fit a 60-minute booking. Choose 30 minutes for a short follow-up.

Honest limits: I cannot see how someone walks into the building or looks in the waiting room, exam-room audio is sometimes poor, and some practices require written consent before a third party joins. If physical presence is what your person needs, book in-person instead. I will tell you if I think that is the case.

In-Person Accompaniment & Care Summary

Five counties

I meet you there. Specialist consultations, care conferences, discharge meetings, second opinions, and any appointment where the information matters and one person cannot hold all of it.

What is included, $100: a prep call to build your question list, two hours with you at the appointment, notes taken throughout, questions asked on your behalf, and a written care summary of what was said, what was decided, and what happens next.

Beyond two hours: $50 per additional hour.

Why the summary is included: The written summary is what you still have next week, and what you can forward to a sibling who could not be there. That is why it is part of the price.

Where: Montgomery, Chester, Delaware, Bucks, and Philadelphia counties. Mileage included within 20 miles of King of Prussia. Please book at least 48 hours ahead so there is time for the prep call.

From $1002-hour minimum, then $50 per added hour

I don't know what we qualify for

What applies to you, and what to apply for first.

Benefits Review & Optimization

Video or phone

A systematic look at Medicare, Medicaid, VA benefits, and long-term care insurance to find coverage nobody has claimed.

What you get: a review of what your person is enrolled in, what they may be eligible for and are not using, and a written list of what to apply for in what order.

Worth checking: VA Aid and Attendance, long-term care policies that include a care coordination benefit, and supplemental benefits on Medicare Advantage plans.

Bring: insurance cards, any policy documents, and a list of current providers. A 60-minute session is usually the right length.

$55 / 30 min · $100 / 60 min

PA Medicaid Navigation

Video or phone

MAWD, OLTL, and ODP waiver guidance: eligibility, applications, and the mistakes that cost families time.

What you get: a plain-English explanation of which program fits your situation, what the application actually asks for, and a checklist of documents to gather before you start.

Why it matters: PA waiver programs are hard to work out on your own, and mistakes on an application can cost months.

Please note: this is guidance, not legal or financial advice, and Gilgal does not file applications on your behalf.

Work with me directly

Bring any stage: thinking about starting, an LLC and no idea what comes next, or a deficiency letter from the Department that you do not understand.

Home Care Business Consulting

Video or in person

Chapter 611 application guidance, policy review, startup roadmap, pricing strategy, and DOH inspection prep.

Why it is different: most consultants selling home care startup advice have never held a license. This guidance comes from an application that went through, including the parts that came back for correction.

You leave with: a written summary and a specific next-step list, not general encouragement.

Work through it yourself, or have it done

Both bundles are instant downloads. The Done-For-You bundle includes a kickoff call where we tailor everything to your agency.

Home Care Agency Starter Bundle

Do it yourself

The full licensing library as an instant download: policies, forms, checklists, and the order to do them in. You customize the documents to your agency and work at your own pace.

Who it is for: founders with more time than budget who want to understand the regulations as they build, rather than receive a finished binder.

Works in any state. The structure and the documents are portable; you adapt the regulatory citations to your own state's requirements.

What it does not include: customization to your agency, and no kickoff call. If you would rather have that handled for you, the Done-For-You Bundle is the same library, personalized, for the Mid-Atlantic.

Done-For-You Home Care Bundle

Done for you

Every policy exhibit, intake form, service agreement, and staff document your Chapter 611 application needs, customized to your agency and not a generic template.

How it works: you complete a short intake about your agency, and every document is personally customized to your name, address, and operations before delivery.

Your kickoff call is included, so please book it. After you purchase, schedule your Done-For-You Kick Off Call here → That call is where we go through your intake, tailor the documents to how you actually plan to operate, and map your next steps.

Every policy is referenced to the specific regulatory section it satisfies, so when a surveyor asks, you can point to the answer.

Built for the Mid-Atlantic: Pennsylvania, Maryland, Virginia, Delaware, New Jersey, and Washington DC.

Compare: consultants charge $3,000 and up for the same scope. This is $597 because the work is already done and tested.
