For families deciding what care looks like, who provides it, and how everyone stays on the same page. Most families arrive here after a fall, a diagnosis, or a hospital discharge — and discover nobody handed them a plan.

Family Care Planning

A written care plan built around your person — their routines, their preferences, their actual home, and what your family can realistically sustain.

What you get: a documented plan covering daily care needs, safety, medications, appointments, emergency contacts, and who does what. Written in plain language you can hand to a caregiver, a sibling, or a hospital.

Why it matters: care that lives only in one person's head collapses the moment that person gets sick, travels, or burns out. A written plan survives all three.

Best booked as: a 60-minute session. Thirty rarely covers a full plan.

Family Care Conference

The family sits down together, and someone who actually knows what care costs and how it works is in the room. Most family disagreements about care are not really disagreements — they are two people guessing at different numbers.

What you get: a pre-call with the organizing family member to set the agenda, a 90-minute conference in person or on video, and a written record of what was decided and who agreed to do what.

Why it works: one sibling believes round-the-clock care runs $3,000 a month. Another is certain Medicare covers it. Neither is right, and they have been arguing for six weeks on top of that. Put real numbers, real options, and real timelines on the table and a surprising amount of the conflict simply goes away.

What I bring: what care actually costs in this region, what Medicaid and waiver programs do and do not cover, what a direct care worker may legally do, what tends to break down at 2am when nobody has planned for it, and what other families in this position have chosen.

What this is not: I am not an attorney and I am not a certified mediator. I do not decide who is right, and the written record is a summary of your family's decisions — it is not a legal agreement and it binds no one.

When I will refer you elsewhere: contested guardianship or power of attorney, suspected financial exploitation, estate or inheritance disputes, and questions about whether someone still has capacity to decide — those need an elder law attorney or a physician. If the conflict runs deeper than information, a certified elder mediator is the right call and I will say so.

Hospital-to-Home Transition

Everything coordinated before discharge — so coming home is organized and safe from the first hour rather than improvised for two weeks.

What you get: review of the discharge instructions, equipment ordered and confirmed, follow-up appointments booked, provider contact list assembled, home readiness checked, and a written transition plan for the first two weeks.

Why it matters: readmission usually traces back to something ordinary — a prescription nobody filled, a follow-up nobody booked, a shower with no grab bar. This is the service that prevents the second hospital stay.

Timing: most useful in the 48 hours before discharge. Call as soon as a discharge date is mentioned.

Advocacy & Appointment Support

Someone in your corner at the appointment — taking notes, asking the questions you will think of afterward, and sending you a written summary the same day.

Patients who have an advocate involved keep more of their appointments and leave them understanding more. Nobody retains a specialist's instructions while frightened. A second person, whose only job is listening, changes what you walk away with.

Virtual Appointment Support

I join your appointment by phone or video — a telehealth visit, or an in-person visit with me on speakerphone in the exam room.

What you get: a 15-minute prep call beforehand to build your question list, presence for the appointment, and a written summary the same day covering what was said, what was decided, and what happens next.

Why it matters: no travel means no drive time to pay for, and it is available anywhere in Pennsylvania rather than just our five counties. Most appointments fit a 60-minute booking; choose 30 minutes for a short follow-up.

Honest limits: I cannot see how someone walks into the building or looks in the waiting room, exam-room audio is sometimes poor, and some practices require written consent before a third party joins. If physical presence is what your person needs, book in-person instead — I will tell you if I think that is the case.

In-Person Accompaniment & Care Summary

I meet you there. Specialist consultations, care conferences, discharge meetings, second opinions — any appointment where the information matters and one person cannot hold all of it.

What is included, $100: a prep call to build your question list, two hours with you at the appointment, notes taken throughout, questions asked on your behalf, and a written care summary — what was said, what was decided, and what happens next.

Beyond two hours: $50 per additional hour.

Why the summary is included: nobody retains a specialist's instructions while frightened. The summary is the part you still have next week, and the part you can forward to a sibling who could not be there. It should not be optional.

Where: Montgomery, Chester, Delaware, Bucks, and Philadelphia counties. Mileage included within 20 miles of King of Prussia. Please book at least 48 hours ahead so there is time for the prep call.
