Someone hurt, not breathing, or in danger: call 911 first. Then call Gilgal. The client's home binder, Tab 1, says who else to call for that client.

Client care notes

Read your client's recent shift notes so you know how their last shifts went. Enter the client's initials, then on the next page enter the care code Gilgal gave you for that client.

Open care notes

The client care page has its own password. You only see the client whose care code you have.

Every shift

  • Read the client's home binder if anything has changed since your last shift.
  • Do the handoff in person with the caregiver before and after you. Don't leave until your relief arrives. If they are late, stay and call Gilgal.
  • Clock in on the shift note page when you arrive and clock out when you leave, then send your shift note before you go.
  • Call Gilgal the same shift about any fall, injury, new confusion, or big change in eating, pain, skin or breathing.

What you never do

  • Give medication of any kind. You remind; the client takes it.
  • Do nursing tasks, whatever license you hold. Gilgal provides non-medical care.
  • Sign or endorse checks, use a client's bank cards, or take money or gifts.
  • Accept power of attorney or guardianship for a client.
  • Share client information, photos or names with anyone outside the care team, including on social media or group chats.

Before you leave every shift

Shift note

One note for every shift you work. Fill it in before you leave. It goes straight to Gilgal.

Someone hurt, not breathing, or in danger: call 911. Then call Gilgal at (484) 202-7837. Follow the client's home binder for who else to call. This form is not how you report an emergency.

Visit clock-in

Tap Start when you arrive and End when you leave. This records the time and your location so Gilgal can confirm the visit, and it fills in your arrival and departure times below. Allow location when your phone asks.

Shift

Initials only. Never the client's full name.

Write your actual times. These records are used for payroll and billing.

Overnight

This appears for an awake overnight shift. Check only what happened during the night. If you check a box, add a short note for it.

Overall sleep
Check what happened. A checked box needs a note.

A quick timestamped line for each check or waking. This is the clearest record of the night.

Falls, injuries and refusals

Was there a fall, near fall, injury or 911 call this shift?
Call Gilgal now if you haven't already, and fill out the paper Incident Report in the home binder before you leave.
Did you call Gilgal about it?
Did the client decline any care?

End of shift

Write what you saw and heard, not opinions. "Ate two bites of lunch" is better than "didn't eat well."

Was the shift completed as scheduled?

Gilgal gives you this code. Don't share it.

Already sent a note with a mistake, or left something out? Use Amend a note. Don't send a second note for the same shift.

Videos, rules and your written test

Training

Gilgal's direct care worker training meets 28 Pa. Code § 611.55. You are not assigned to a client until your competency is documented.

How the program runs

  1. Orientation. Mission, how Gilgal works, policies, reporting, and your paperwork.
  2. Three self-paced days. Watch the videos for Day 1, Day 2 and Day 3 below, in order, on your own time. Read the Gilgal rules under each topic. They are what the test covers.
  3. Written test, online. Take it on the Written test page when you finish the videos. Your answers go to Gilgal.
  4. Competency check with Gilgal. Skills you show the evaluator, role-play and a documentation exercise. You get your certificate when you pass every part.
  5. First week with a client. A supervisor or senior caregiver works alongside you and confirms your skills in the client's home before you work on your own.
  6. Every year after that. A competency review, and sooner after any quality-of-care discipline.

Companion workers need Day 1, Day 2 and the first topic on Day 3. Personal care workers need everything.

About these videos. They are free public videos from the CDC, the USDA, other government agencies, university health systems and nursing assistant programs. Some were filmed in nursing homes, so the beds and equipment will look different from a client's home. Where a video and the Gilgal rules on this page differ, follow Gilgal. Tick "Watched" as you go — it saves on this phone or computer only, so you can pick up where you left off. It is just for you and is not sent to Gilgal. When you finish, send the training confirmation lower on this page so Gilgal has it on record.

Confirm you finished the videos

When you have watched all the videos for your level and read the Gilgal rules under each topic, confirm it here. This goes to Gilgal for your training record. You still take the written test and do the competency check with Gilgal.

Your level *

Not sure? Ask Gilgal before you confirm.

Competency check

  • A written test on every subject area that applies to you, taken online. You need at least 3 of 4 questions right in each area.
  • Skills you show the evaluator: handwashing, putting on and taking off gloves, and a transfer with a gait belt.
  • Two short role-plays: a difficult behavior, and a situation where you may need to report abuse or neglect.
  • Writing a shift note from a practice scenario.

Gilgal will tell you the date and place for the skills check. If you don't pass an area, you review it and try that area again before you are assigned to a client.

Take the written testOpens on its own page with its own password. Closed book. Your answers go to Gilgal.
