Two ways to bring care home. We are licensed for both.

Gilgal holds a Pennsylvania license as both a home care agency and a home care registry. Because we are not limited to one model, the conversation starts with what fits your family, not with what fits our business.

Non-medical support at any age. We do not provide skilled nursing, therapy, or medication administration — if that is what is needed, we will help you find the right provider.

What It Costs

Same caregivers, same clearances, same standards. What changes is who employs the caregiver — and that changes who you write the check to, who carries the risk, and who directs the work.

Gilgal employs the caregiver

One hourly rate. One invoice, from us.

The caregiver is our employee. We recruit, clear, train, supervise, and pay them, and we carry payroll taxes, workers' compensation, and liability coverage. You are billed a single hourly rate and never become anyone's employer.

Starting rates. Final rate depends on schedule, acuity, and location, and you are always quoted before anything is booked. No setup fee and no separate placement charge — the hourly rate is everything.

Support during cancer care is not automatically a specialized-tier service. What is needed varies enormously from one person to the next, so we work the level out with you rather than assuming it.

Care clients also receive a home safety walkthrough, and transition support after a hospital stay, at no additional charge — see what comes with care.

Steady Hours. Three hours per visit, nine hours per week, at any level of care. Enough for a morning routine, an evening, or a few hours so you can be somewhere else.

Short daily visits. If what you need is a Morning Start, an Evening Settle, or a Daily Check-In, the minimum is two hours per visit and eight hours per week — at the same rates above.

You hire the caregiver directly

Family-Led Care

You choose the caregiver. You agree on the rate. You pay them directly.

Family-Led Care runs through our licensed home care registry. You hire your own caregiver. We refer independent contractors from our cleared roster — every one vetted to the same standard as our own team — and you choose who you want. If there is someone you already know and would rather have, we can run them through the same vetting and add them to the roster.

From there, you and the caregiver agree on the hourly rate between yourselves, and you pay them directly. Gilgal does not set that rate, does not publish or recommend it, does not collect it, and is not the caregiver's employer. What we charge is separate, and it is the only money that comes to us.

What we handle is the part families should not have to manage alone: the background clearances, reference checks, competency verification, rostering, and the Chapter 611 documentation that keeps everything above board. You keep the relationship and the scheduling.

Why families choose Family-Led Care
  • You choose the person — including someone you already have. If there is a caregiver your family already knows and trusts, we can vet them and add them to the roster. Under the agency model that person would have to become our employee; here they stay yours.
  • The vetting is finished before you ever meet them. Clearances, verified references, competency, and the Chapter 611 file — the work most families hiring privately never do, and often do not know is required.
  • You keep the schedule and the relationship. Visit lengths, days, hours, and how the work gets done are yours to set with the contractor. We do not direct any of it.
  • Our fee does not change with the hours. Placement is charged once. The daily fee is the same whether your contractor comes for two hours or stays overnight.
  • You are not starting over if it ends. If the arrangement stops working, we can refer another cleared contractor from the roster rather than sending you back to square one.
  • You keep the paperwork. Clearance certificates, reference and competency records, and the required disclosure of the contractor’s independent status are handed to you, not just filed with us.

The daily fee applies only on days your contractor actually works. If care pauses for travel or a hospital stay, the fee pauses with it. Other fee structures — hourly, weekly, monthly, or a single flat referral fee — are available by mutual agreement; ask on your placement call.

Scheduling under Family-Led Care is arranged directly between your family and the contractor. Visit lengths and weekly hours are yours to set together — Gilgal does not set them, and the agency visit minimums above do not apply here.

Family-Led Care is our name for the registry model. It is similar in spirit to the self-directed arrangements some families know from Medicaid waiver programs, but it is private pay and is not a Medicaid program.

What the caregiver charges is between you and them. Gilgal does not set, publish, recommend, or negotiate that rate, and we never collect it. You and the contractor agree on it directly, and you pay them directly. If you would rather not carry that conversation at all, that is exactly why the agency model exists.

Honest about what this asks of you. Because you pay the contractor directly, the working relationship sits with the two of you — the schedule, coverage when they are unavailable, and any tax reporting that applies to paying someone directly. We will tell you plainly what we know and point you to a tax professional for the rest. If that is more than you want to carry right now, the agency model is the simpler path.

Before you look at rates. Most families ask about Medicaid first and find out they don't qualify. If that is you, there are more options than people realize — VA benefits, long-term care insurance, spend-down, and ways to make private pay work for far less than you would expect. Home Safety Walkthrough

Included

A room-by-room walkthrough with a written report listing every hazard by risk level, and an estimated cost for each correction. Done as part of starting care, not as a separate purchase.

Hospital-to-Home Support

Included

Discharge instructions reviewed, equipment confirmed, and follow-ups booked, plus a written plan for the first two weeks home. Included when care begins within 30 days of a hospital stay.

Family Care Planning

$55 · $110

A sit-down to work out what is actually needed and what it would take — useful whether or not you go on to use us. Thirty minutes at $55, or a full hour at $110.

Every caregiver completes a Pennsylvania State Police criminal history clearance before entering a home, along with FBI fingerprint and ChildLine child abuse clearances where Chapter 611 requires them. Under the registry model, Gilgal refers independent contractors and does not employ, supervise, or set the compensation of the contractor you select.
